Amador Valley High School - Class of 2025 Grad Night Consent Form

ALL FIELDS ON THIS FORM ARE REQUIRED

Grad Night 2025 is a parent-sponsored activity, NOT an Amador Valley High School (AVHS) event.  Students are expected to follow school rules regarding behavior.  Please review this consent form with your student before you BOTH sign it.  Every student must have a signed consent from on file in order to attend Grad Night.  Additional consent forms may be required from other suppliers of the event.

THINGS TO REMEMBER:
  • Grad Night 2025 will be held on May 30, 2025, 10:30pm to May 31, 2025, 1:30am.
  • Student ID will be required for entrance.
  • All students will ride the bus to and from Grad Night at Plank Oakland.  Students may not drive themselves.
  • All students must check-in for bus transportation by 9:30pm. LATE ARRIVALS WILL NOT BE ADMITTED — STUDENTS MUST BE ON THE BUS TO ATTEND.
  • If a student decides to leave Grad Night 2025 before the bus departs to return to Amador Valley High School, the student must be picked up by a parent (this should be for illness only).

Student / Parent Agreement

I hereby give my permission for my student (named above) to participate in Grad Night 2025.  I understand this is not a Pleasanton Unified School District (PUSD) or AVHS sponsored event and hereby release the parent organizers, PUSD and AVHS from all liability in connection to Grad Night 2025  In the event of a sudden illness or accident the sponsor has my permission to render emergency medical treatment deemed necessary for the above named student.

I understand that of safety is of utmost importance at Grad Night 2025 and I agree to abide by all school rules regarding behavior.  If I am found with or it is determined I am under the influence of illegal substances and/or alcohol before or during the event, then my parents and/or police will be called, I will be required to leave the Grad Night event and will not be allowed to return.  If this occurs, no refunds will be issued to me.  Additionally, if I decide not to attend Grad Night 2025, my payment will not be refunded.  I also understand that while the volunteer staff will attempt to maintain security in the room where personal property that belongs to me may be checked, the Senior Class Parent Committee members and volunteers cannot be held liable for personal belongings.  Participants will be subject to search.

Acknowledgement of Risks, Assumption of Risks,
& Responsibility of Liability for Organizations and Individuals


Warning:  There are significant elements of risk in any adventure, sports or activity associated with the indoor or outdoor use of games and activities incidental thereto (referred to herein as "activity").  Although we have taken reasonable steps to provide you with the appropriate equipment and/or skilled staff, this activity is not without risk.  Certain risks cannot be eliminated without destroying the unique character of the activity.  For your safety, and that of other participants, we think it is important for you to know in advance what to expect and to be informed of the inherent risks. 

Acknowledgement of Risks:  I acknowledge that the following describes some, BUT NOT ALL, of those risks: risks of personal injury, accidents and/or illness, including but not limited to sprains, torn muscles and/or ligaments, fractured or broken bones, eye damage, cuts, wounds, scrapes, abrasions and/or contusions, dehydration, oxygen shortage (anoxia), and/or exposure, head, neck, and/or spine injuries, allergic reaction, shock, paralysis or death.  I/We understand the description of these risks is not complete and that unknown or unanticipated risks may result in injury, illness or death.

Express Assumption of Risk and Responsibility: I/we agree to assume responsibility for the risks identified herein and those risks not specifically identified.  My/our participation in these activities is purely voluntary.  No one is forcing me/us to participate.  I verify that I am physically fit, not under the influence of alcohol or drugs at this time, and sufficiently qualified and trained and capable to participate in these activities.  I/we elect to participate in spite of the risks.

Authorization: I/we hereby authorize any medical treatment deemed necessary in the event of and injury while participating in the activity.  I/we agree to any film or photographs of me/us as a participant becomes your property and may be used for promotional purposes.

Release: In consideration of services or property provided, I, for myself, my employees, and/or any minor children for which I am parent, legal guardian, group leader, or otherwise responsible, any heirs, personal representatives or assigns, agree that Plan-It Interactive, its principals, directors, officers, agents, employees and volunteers, their insurers and each and every land owner, municipal and/or governmental agency upon those property an activity is conducted and their insures, if any, shall have no liability expect for its sole negligence.

I have read the Acknowledgement of Risk, Assumption of Risks and Responsibilities, and Release of Liability.
